“According to Art. 13 sec. 1 of the General Data Protection Regulation (GDPR, RODO), we would like to inform you that:
The administrator of personal data is the National Institute of Geriatrics, Rheumatology and Rehabilitation named of prof. dr hab. med. Eleonora Reicher, ul. Spartańska 1, 02-637 Warsaw;
The administrator has appointed a Data Protection Officer with whom I can contact regarding the processing of my personal data via e-mail: kancelaria@spartanska.pl;
The administrator will process your personal data on the basis of art. 6 sec. 1 lit. b) GDPR, i.e. processing is necessary for the performance of a contract to which the data subject is party or to take steps at the request of the data subject prior to entering into a contract;
Personal data may be made available to other authorized entities, on the basis of legal provisions, as well as to entities with which the Administrator has concluded a contract in connection with the provision of services to the Administrator (e.g. a law firm, software provider, external auditor);

You have the right to obtain a copy of your personal data at the Administrator’s office.
Additionally, in accordance with Art. 13 sec. 2 GDPR, we inform you that:
Your personal data will be stored for a period of 10 years from the end of the calendar year in which the contract was performed, unless a longer processing period is necessary, e.g. due to pursuing claims;
You have the right to access your data, rectify it or limit its processing, as well as the right to object to the processing, the right to transfer data and the right to lodge a complaint with the supervisory body, i.e. the President of the Office for Personal Data Protection;
Providing personal data is voluntary, but necessary to conclude a contract. Failure to provide personal data will result in failure to perform the contract;
The administrator does not make automated decisions based on your personal data. „
